Your application to end a civil partnership is now ready to be downloaded and printed.

1. Download your application form

You can print the application form, or you can save it and print it later.

2. Complete the statutory declaration(s)

Visit a Justice of the Peace or another authorised person along with your proof of ID documents to be certified and to complete the statutory declaration section of your application form.

If your partner is not signing the application form, you must also complete the statutory declaration for a single applicant and submit it with your application form.

3. Submit your application

Submit by post

Post your printed application form, certified copies of your proof of ID and payment to���Registry of Births, Deaths and Marriages, PO Box 15188, CITY EAST QLD 4002.

Submit in person

Take your application form, original proof of ID and payment at the Brisbane registry customer service centre at Level 32, 180 Ann Street, Brisbane.
