COVID-19 virus implications

Please note that the COVID-19 virus is currently affecting agencies’ ability to obtain records and deal with applications under the Right to Information Act 2009 and the Information Privacy Act 2009.

Before you make an application, we recommend you contact the relevant agency.

Right to Information application form

This is the approved form for making online Right to Information (RTI) and Information Privacy (IP) applications.

Applications requesting access to documents that do not contain personal information either in relation to themselves or another person on whose behalf they are applying are made under the Right to Information (RTI) Act 2009. There is an application fee for access to documents, as well as processing and access charges.

Applications requesting access to personal information only are made under the Information Privacy (IP) Act 2009. There is no application fee or processing charges for access to your own personal information only, however there may be access charges. You will be required to provide evidence of your identity before personal information can be released.

Only requests to Queensland Government departments and Ministers can be made through this application form. You must complete a separate form for each department. Each application made under the RTI Act incurs a separate application fee.

Do not use this form to request documents from statutory authorities, local governments, government owned corporations or public authorities (such as universities); you will need to submit an application by filling out the printed form and submitting it.

    Applications for your own personal information may be processed under the IP Act or the RTI Act, depending on the nature of your application. For example, it will be an RTI application if you are also applying for:
    1. someone else's information, and you are not seeking access on that person’s behalf
    2. documents that don't contain your personal information
    You will be advised by the RTI officer if this is the case and you will be required at that time to pay the RTI application fee. If in doubt, contact the RTI officer of the agency to which you are applying.

    You will be required to provide proof of your authorisation to act on the person’s behalf, such as legal documentation in support of your authority (for example: a client agreement if you are a solicitor) or written authorisation from the person concerned.
